Objectives
As a result of this activity, participants will be able to:
- Discuss the unique challenges faced by adolescents and young adults with sickle cell disease as they transition from pediatric to adult care.
- Describe the current landscape for transition support interventions and identify at least one relevant approach to their clinical practice.
