Objectives
- Describe the role of the clinical documentation integrity team in creating notes that are reflective of a patient's clinical status during the hospitalization.
- Recognize areas in their documentation that can be improved upon to support medical necessity, defend against future audits and adequately translate clinical terms into ICD-10 diagnoses necessary for coding and billing.
