Objectives:

The participants will 

  • Relate an understanding of the required documentation for advance directives 
  • Identify best practices for initiating sensitive conversations 
  • Describe strategies to ensure that care preferences are clearly communicated and honored throughout the care continuum

This lecture will explore the critical role of advance directives within pediatric care, emphasizing their importance in supporting informed decision-making for patients and families. The session will address key components of completing advance directives, including the unique considerations that arise as adolescents approach adulthood and transition from pediatric to adult healthcare systems. Participants will gain an understanding of required documentation, best practices for initiating sensitive conversations, and strategies to ensure that care preferences are clearly communicated and honored throughout the care continuum
